To specify, we collect pertinent patient information such as the following: demographic details such as an upload of guardian/ patient’s government ID for verification purposes, patient’s full name, residing address, age, sex, marital status, contact details such as email and contact number, birthdate; referring physician, patient diagnosis and an upload of the patient’s prescription; work information such as: occupation, address of the occupation, and work number; Emergency information such as the person to contact, their cellphone number, and relationship to the patient; Payment details such as the person responsible for claiming invoices and receipts, their cellphone number and mode of payment; others such as comments, feedback, service reviews, recommendations and personal profile.

 

When you conduct a transaction on our website, as for our protocol, we collect personal information you give as specified. The information you provide will be used for specific reasons such as:

2. To provide our patients and/or their family member/ guardian in charge with ongoing home healthcare services;

Med-Easy Home Healthcare Services is hosted on the Wix.com platform. Wix.com provides us with the online platform to allow us to provide our home healthcare services to you. Your data may be stored through Wix.com’s data storage, databases and the general Wix.com applications. They store your data on secure servers behind a firewall. 

 

All direct payment gateways offered by Wix.com and used by our company (specifically dragonpay) adhere to the standards set by PCI-DSS as managed by the PCI Security Standards Council, which is a joint effort of brands like Visa, MasterCard, American Express and Discover. PCI-DSS requirements help ensure the secure handling of credit card information by our store and its service providers.

 

We may contact you to notify you regarding the home healthcare service you availed, to notify you whether the therapist and/ or nurse is on their way to your home, any concerns regarding collecting fees or monies owed, to poll your opinions through surveys or questionnaires, to send updates about our company, or as otherwise necessary to contact you to enforce our User Agreement, applicable national laws, and any agreement we may have with you. For these purposes we may contact you via email, telephone and text messages.
